How to complete your Pledge Form...

  • Fill in your personal information: Please provide a daytime phone number. It will be helpful to the CFC Key Workers and Project Officers in case there is a question about your pledge card.
  • For payroll deduction giving: In the AMOUNT column, write the amount of your gift per pay period or per month.
  • Under TOTAL GIFT, write in the annual amount of your gift.
  • Sign and date the pledge card if you are pledging by payroll deduction: Provide your Social Security Number (SSN) or Employee ID in the specified block. Your SSN or Employee ID will only appear on the Payroll Office copy of the pledge form.
  • If your are giving by cash or check, no SSN or Employee ID is required.
  • Minimum amount limit: The minimum payroll deduction per pay period for civilians is $1.50. The minimum payroll deduction per month for military is $2.00.
  • For cash or check donations: Circle either Cash or Check and write in the amount of your gift. Please make checks payable to CFC or Combined Federal Campaign.
  • Designating your gift: Fill in the five-digit code number of the charity or charities you select. There is an alphabetical index at the back of the brochure which lists all the charities, and their five-digit codes, which are participating in our campaign.
  • Under ANNUAL AMOUNT, write in that portion of your total gift which you are designating to a specific charitable organization. If you designate multiple charities, please assure that the sum of the individual donations equals your total annual donation.
  • No five-digit code write-ins, please: CFC regulations prohibit write-ins of charities you may prefer. You may only designate charities listed in this brochure. CFC regulations state that designations to unlisted organizations will automatically become “undesignated” donations.
  • Recognition Options: This block permits you to enter your name and home address, or home e-mail address, should you wish to authorize the CFC to release your personal information to the charity/charities that you designate on this form. Without your authorization, the CFC is prohibited from releasing your name to the organization(s) you support.
  • If you also wish the CFC to release the amount of your pledge(s) to your designated charities, you must check the “Yes” box at the bottom of the Recognition Options box.
  • Copies of the pledge form: Detach copy #3 (pink copy) and retain for your personal records. Return the cover page, the original (white copy), and copy #2 (yellow copy) to your Key Worker or campaign solicitor.
  • Leadership Givers: If you are a leadership giver – Bronze, Silver, or Gold Eagle – circle the gift you qualify for and would like to receive.
